Community-based subacute care

Community-based rehabilitation

Community-based rehabilitation is available from Community Physiotherapy Services in the Perth metropolitan area.

The programs are designed to help you maximise your functional ability and manage the impact of your condition.

They include physical activity as well as education programs for adults with a chronic condition.

The programs are group based and take place in community settings such as at recreation or senior citizen centres.

There is no cost involved, but you will need a referral from your general practitioner or hospital allied health professionals.

Community-based physiotherapy – Northam

In Northam a community-based physiotherapy service provides:

  • gym rehabilitation
  • group programs for cardio or orthopaedic rehabilitation.

The physiotherapist also visits York, Cunderdin and Toodyay for weekly clinics.

Community-based palliative care

Community-based palliative care services are provided by Silver Chain (external site) across the metropolitan area and in some rural regions.

There is no cost involved, but you will need a referral from your general practitioner or a doctor at your hospital.
