How do I register my child/dependant to receive their vaccines at school?

Please use the secure online consent form. For further information, visit School-based immunisation program and see the “How to register your child and provide consent” section.

Please be aware: if your child attends a school on Cocos or Christmas Island or within the City of Joondalup, please use a paper form. If your dependant is a child under the care of the Department of Communities, please contact your dependant’s case worker to complete a form and return it to their school.

I don’t want my child/dependant to receive their vaccines at school. Do I still need to complete the consent form?

The option to decline is available on the online consent form, and we request that the consent form is completed to provide declination, as well as consent. This assures that the SBIP nursing staff are aware of your choice, and you will not receive a follow-up call. Your dependant will not receive any vaccines if you have selected this option.

If you do not consent to receiving any of the listed Vaccination Plans, you will be asked to select the applicable reasons for declining the vaccination. Once you have clicked the ‘Submit’ button at the bottom of the page, you will receive a message confirming that you have not consented for your dependant to receive the vaccination. They will not receive the school-based vaccinations.

How many dependants can I register under my account?

There is no limit to the number of dependants that you can register under your account.

I would like my dependant to log in to and manage their own account. Is this possible?

A ‘dependant’ includes anyone who relies on a parent or legal guardian for support, including children under the age of 16. The parent/legal guardian is required to manage the dependant’s account and consent responses on behalf of their dependant/s, who will not be able to register themselves on VaccinateWA, log into, or manage their own account. Anyone who is not a dependant should create their own account.

If you would like for your dependant to receive communications from the system directly, you can update their email and phone contacts. Please note that in this instance you will no longer receive correspondence.

If your dependant is a child, they can have their own account once they turn 16. To request this, please contact SBIP@health.wa.gov.au.

I would like to transfer the guardianship of a dependant to another person. Is this possible?

Yes, please email SBIP@health.wa.gov.au.

I would like SMS and email reminders to be sent to my dependant instead of myself, is this possible?

Yes, please email SBIP@health.wa.gov.au.

Can I change my mind once I have provided my response?

Yes, you can change your response by resubmitting the online consent form.

Where can I find more information around the SBIP?

Visit School-based immunisation program for information on where to get help, including contact details for your area.
