The End-of-Life Care Program wants your say in the development of the WA Advance Health Directive Guide.
The Guide will accompany the updated WA Advance Health Directive (AHD) and will instruct people on how to complete the new AHD.
Community consultation about the development of the new AHD Guide is happening from 29 November 2021 to 17 December 2021 through a series of workshops and an online survey. Anyone interested in advance care planning is invited to participate.
Consultation workshops – register now
The workshops offer an opportunity for stakeholders to review the Guide, have face-to-face discussions and provide feedback into the development of the new AHD Guide. There are a range of workshops for specific groups, register or learn more via the links below:
Targeted consultations will be held for people from Aboriginal communities.
Online survey
The survey will be open from 31 January 2022 to 18February 2022 through an online survey and a series of workshops. and take approximately 20 minutes to complete.
